Explanation of Worksite Forms
During the first seminar meeting you will be given three copies of the Agreement between you, the worksite, and Sauk Valley Community College. The purpose of the agreement is to provide a clear understanding to all participants of their responsibilities during the practicum experience.
The first day of work, you and your worksite supervisor will review the worksite, the SVCC and student agreement form. Once everyone understands and agrees to the conditions, you and your supervisor will sign the forms. On the assigned date you will return all three forms to the SVCC Coordinator, who will also sign the forms. The Coordinator will keep a form, you will keep a form, and the worksite supervisor will keep a form.
The first meeting you will also receive a letter for your work sire supervisor from the SVCC coordinator. The letter addresses other issues the worksite may have concerning the practicum. This letter must be given to the worksite before or during your first day of work.
You will also receive a Form entitled, Practicum Goals Record. With the help of you worksite supervisor, you will list two or three goals you hope to accomplish during the semester. You will also list how you interned to accomplish the goals. You will return this form back to the SVCC coordinator on the assigned date. The Coordinator will review the goals, and return them to you at the next meeting. You will use these goals as an evaluation of your achievement.
Due dates for all the forms will be given to you at the beginning of the semester. It is your responsibility to make worksite supervisors aware of due dates. Your final grade will be affected if forms are not turned in on the assigned date.
