Worksite Evaluations

 

Each semester two student evaluations are required, one at midterm and one for the finals.  At the beginning of the semester you will receive two copies of each evaluation form.  One copy is for you to independently complete, and the other form is for your supervisor to independently complete.  After you have both completed the evaluations, you and the supervisor must meet to compare and discuss similarities and differences between the evaluations.  Both the supervisor and the student sign and date both forms and return both forms to the SVCC Coordinator on the assigned date.

 

At the end of the semester, the student will also complete and evaluation of the worksite.  You will assess the worksite’s helpfulness in preparing you for your chosen profession.  This evaluation will be utilized by the SVCC Coordinator to determine if other students would benefit from placement at the worksite.  The evaluation will be turned in on the same date as the student’s final worksite evaluation.  Exact completion date will be announced at the beginning of the semester.

            Overall, has the student stayed the same, improved or greatly improved during his/her time with you? Please explain and give a brief example.

 

The areas the questions cover include knowledge of setting, interaction with others, ethical awareness and conduct, performance of basic work requirements and response to supervisor.  A checklist enables the responder to circle the correct answer in the following manner:

           

            N/A-    Not applicable or not enough information to form a judgement

1-                 Far below expectations –needs much improvement, a concern

2-                 Below expectation- needs some improvement to meet standards

3-                 Acceptable- meets standards at average for a student

4-                 Above expectation- performs above average level for a student

5-                 Far above expectations- a definite strength, performs well beyond average level for a student
