Crusader Clinic utilizes board-certified physicians, nurse practitioners, and physician assistants in the delivery of a full-range of medical services. Medical care is available in the areas of family practice, internal medicine, women's health and pediatrics. Specialty medical services include podiatry, obstetrics, endocrinology and orthopedics. You may be eligible for a discount if you are not covered by an insurance plan. Discounts are based upon family size and income. Proof of income is required to be considered for a discount. Discount levels are 25%, 50%, 75% and 100% on eligible services. There is a minimum $20 co-payment due per medical visit and a $30 co-payment due per dental, optical, or podiatry visit, regardless of discount. These payments are due at the time of service. Discounts are re-evaluated annually. Some patients may be over the income level for a discont, but can still receive services at Crusader Clinic. Patients whose income qualifies them for a 75% or 100% discount may be required to be assessed for eligible entitlements (entitlements may include Medicaid or other public benefits), and may be required to apply for assistance before being considered for a discount. Crusader Clinic provides assistance in completing all entitlement applications. Most laboratory and radiology services for self-pay patients ordered and processed at the clinic are eligible for discounts. Laboratory and radiology services for patients with third-party payers are billed by the clinic's contracted vendors. Some Crusader Clinic services are ineligible for a discount. These services include prenatal and dental procedures involving prosthetics (dentures & partials) and root canals. Eyewear and optical products are also ineligible for discounts.
