Title IX Incident Reporting Form

Title IX Report

This form will be electronically submitted to the Title IX coordinator and time/date stamped.  Institution will respond to complaint within 12 business hours.

Person reporting the incident can be a member of the bystander group defined as either witnessing the incident or hearing of such incident.  Reporter should gather as much information on those parties with knowledge of the incident to pass on to the Title IX Coordinator for investigation.
