Responsibilities
While Care Coordinators are trained and expected to serve in all Care Coordination Unit capacities, the main job focus of this position is to assess and authorize Community Care Program services as well as provide ongoing case management services to both CCP and Title III eligible participants throughout The service region of Whiteside, Carroll, Ogle, and Lee Counties, IL.

1. Receive requests for CCP assessments from clients, family members, and community partners.
2. Use Comprehensive and Redetermination Assessments to make an official eligibility determination within 30 days of the request for services.
3. Complete all annual redeterminations within 1 year of the most recent assessment.
4. Complete 6-month FTF follow up.
5. Provide ongoing case management services to both CCP participants and Title III (Non-CCP) Case Management participants.
6. Maintain ongoing contact with program supervisor while in the field and coordinate hard to reach clients/service delivery through the program supervisor.
7. Complete CFC duties as assigned by program supervisor.
