Purpose:
Child & Family Connections serves as the single access point to Early Intervention services. Early Intervention services are for families who have infants and toddlers up to age 3, who have concerns about how their child is developing.
Remarks:
Also serve DeKalb County. For other counties, call (800) 323-4789. All referrals are made to the Central Office. Hours are flexible, most often from 8:00 AM to 4:00 PM, but families can be seen on evenings and weekends. When there is a concern in regard to a child's development, a referral can be made to Child & Family Connections. A service coordinator will meet with the family to discuss the concerns in regard to their child. The child will then receive free evaluations through Early Intervention (EI) to determine eligibility for ongoing services. When a child is found eligible for EI services, an Individualized Family Service Plan (IFSP) may be written. All Early Intervention services are voluntary. Early Intervention services may include assistive technology, audiology/aural rehabilitation, counseling, developmental therapy, family training and support, health services, medical diagnostic services, nursing services, nutrition services, occupational therapy, service coordination, social work, speech/language therapy, transportation, and vision services. Free services include service coordination, evaluations/assessments, interpretations, translation and IFSP development. After elegibility is determined and an IFSP is written, here may be a family fee assessed based on a sliding scale based on household size and gross income. This program is for children, birth to three years old, with suspected developmental delays or a medical diagnosis of a condition that has a high probability of resulting in a developmental delay.
