Review and Resolution of Accommodations
Sauk Valley Community College is committed to providing equal education opportunities for persons with disabilities, and does so in accordance with the Nondiscrimination Policy of Sauk Valley Community College, and in compliance with Section 504 of the Rehabilitation Act of 1973 and with Title II of the Americans with Disabilities Act of 1990 as amended. In compliance with federal mandates, Sauk Valley Community College employs a Coordinator of Disability Support to determine appropriate accommodations for students with documented disabilities. If a faculty member disagrees with the accommodations provided based upon an alteration of academic content, it is the faculty members’ right and responsibility to contact the Coordinator of Disability Support immediately. If the faculty member and the Coordinator are unable to resolve the complaint, the faculty member should complete a Review and Resolution of Accommodations form.
The claim must be based upon a fundamental academic alteration in course materials and not the disability. Disability information is not routinely released to faculty or staff members. “According to the Association on Higher Education and Disabilities (AHEAD), Disability related records provided by a physician, psychiatrist, psychologist, or other recognized professional are not subject to free access under FERPA.” Following best practices established by AHEAD, faculty members are sent a notice of approved accommodations as "it is only necessary to share with the faculty the information that a student has a documented disability and need for accommodation(s)." Faculty members have no need to know the nature of the disability, "only that it has been appropriately verified by the individual (office) assigned this responsibility on behalf of the institution."
The Review and Resolution of Accommodations form will be reviewed and the faculty member will be contacted by the Academic Vice President and/or the Dean of Instructional Services within 5 business days of the filed Review and Resolution of Accommodations. The Vice President or Dean will meet with the Coordinator of Disability Support to discuss the filed Review and Resolution of Accommodations. If no resolutions can be reached to the satisfaction of all parties, the filed Review and Resolution of Accommodations will be submitted to the Affirmative Action Officer, 815 835 6291, located in room 3L20. A formal Review and Resolution of Accommodations committee will be convened to review the complaint and a written response will be provided to all parties within 10 business days from the date the Review and Resolution of Accommodations was submitted to the Affirmative Action Officer.
The faculty member must provide all allocated accommodations as stated in the accommodation notice in dispute during the Review and Resolution of Accommodations procedure. Delays in the implementation of accommodations may be construed as a discriminatory act. Careful consideration will be given to all filed Review and Resolution of Accommodations. No retaliation or intimidation of any person involved with the Review and Resolution of Accommodations procedure will be tolerated and is in direct violation of College policy.

I do not agree to the provision of the attached list of accommodations (s) mandates as outlined in Subpart E of Section 504. I have discussed my concerns with the Coordinator of Disability Support, and we are unable to come to an agreement regarding the accommodation (s) in question. I maintain and can substantiate that the provision of the accommodation (s), as stated, pose a fundamental alteration to the academic curriculum of the course being taught. I wish to initiate Review and Resolution of Accommodations procedures. I understand that I must provide all allocated accommodation(s) as stated in the accommodation notice in dispute during the Review and Resolution of Accommodations procedure.
Please state the specific Review and Resolution of Accommodations with the accommodations as it pertains to the alteration of the mastery of academic curriculum. Attach all documentation to substantiate the alteration.
